If you are concerned that you may have an eating disorder, this questionnaire can serve as a guide to help you understand whether you should talk to a doctor.

Our Eating Disorder Questionnaire is an adaptation of the SCOFF questionnaire, which is a simple five-question screening test. These questions are part of a standard assessment used by doctors to evaluate a person’s risk for an eating disorder.
